Accommodation/Tenancy Assistance
The provision of transition supports for people seeking a place to live, which suitably caters to specific individual needs.
Assistance with Daily Personal Activities (High-Intensity)
Supports are provided by staff who are well experienced and qualified.
Assistance with Life Stage Transitions and Support
We provide mentoring and peer support for individual skill development ad decision making skills for individuals to meet their goal and aspirations in transitional phase of individual’s life.
Assistance with Daily Personal Activities
We provide standard support for ADLs including personal hygiene, grooming, medication assistance, improving communication skills and enhancing better lifestyle.
Assistance with Travel and Transport
Provision of travel arrangements and support for people to access essential services as per specific needs.
Community Nursing Care
This includes providing specialist nursing care in the home for those who require a higher level of care and support.
Assistance with Daily Tasks in a Group or Shared Living
We provide support with daily tasks in a group or shared living environment without compromising the needs of any individual.
Innovative Community Participation
All Nations Care Services support individual for community access in a regular basis to enhance active community participation.
Development of Daily Living and Life Skills
Our service delivery focuses on developmental activities and training to enhance person’s ability to live as independent as possible in everyday life.
Assistance with Household Task
At All Nations Care Services, we provide support and assist individuals with meal preparation, delivery, house or yard maintenance, cleaning, laundry and general maintenance.
Participation in the Community
Our services are tailored to provide support you to go shopping, attend medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and transport, and learn great skills.
Plan Management
Provide supports to manage your funding in NDIS plan. Plan manager assist in maintaining records, managing the NDIS claims, disbursing funds and financial statements status of participant’s plan.
